NATIONAL SCHOLARSHIP PROGRAM
Instructions for Applying Online
If you begin your application online and find
you need to stop mid process, you will be able to return
to complete the process or make changes if necessary
by logging back into the system. Information for each
section is only saved once you select the ‘Save and
Continue' key within that section.
STEP 1: You will be requested
to provide the following information online:
--Social Security Number
--First and Last Name
--Mailing street address or P.O. Box, City, County,
State, Zip
--Home phone number
--E-mail address
--Alternate phone number
--Ethnicity/Nationality
--Place and Date of Birth
--Father's/Legal Guardian's Name
--Mother's/Legal Guardian's Name

--Parents' adjusted gross income
--Total income of Father
--Total income of Mother
--Parents' yearly untaxed income and benefits
--Medical and Dental expenses
--Total cash, checking, savings, and cash value of stocks

--A personal statement that provides information about
your community involvement and desire to contribute
to your community.
STEP 2: Once all information
has been entered:
Ensure all your information is correct;
make any changes if necessary.
Print the Cover Page which will include
the Completion Check List. The Cover Page is
required for the application process in
order for the submission to be complete.
Complete each step of the Completion Check
List.
Make an extra copy of all your documents
and keep for your personal records.
Mail your application with all supporting
documents postmarked by February 15, 2007.
All documents must arrive in the same package
for the submission to be considered complete. Incomplete
applications will not be considered.
To Make Changes to Your Application :
Return to https://www.scholarshipadministrators.net
and click the " APPLY for a new scholarship
" link. Using Login Option 2, enter your
access key ( RMHC ), Application Identification
Number and name to re-enter the application process.
Click on each tab until you reach the section that needs
correcting/updating; make changes as necessary and print
a new Cover Page for your records. You do not have to
resubmit paperwork unless there is a change in test
scores or class rank and/or size, prior to the
Postmark deadline .

Access key RMHC
Background
Local Ronald McDonald House Charities (RMHC) Chapters,
with support from RMHC Global and McDonald's Corporation,
offer scholarships to students from communities who
face limited access to educational and career opportunities.
These scholarships are part of the RMHC and McDonald's
ongoing commitment to education.
History
RMHC began its support of scholarship programs in 1985
, by committing $50,000 toward the first scholarships
awarded through the Hispanic American Commitment to
Education Resources (HACER) program. In 2001, the RMHC/ASIA
(Asian Students Increasing Achievement) and RMHC African
American Future Achievers Scholarships began. To date,
RMHC has awarded more than $19 million to help high
school seniors attend college. RMHC is now broadening
the scope of their national scholarship program. Beginning
in Fall 2005, students of all backgrounds are eligible
to apply for the RMHC National Scholarship Program.

Eligibility
Ronald McDonald House Charities National Scholarships
are open to high school seniors only who are eligible
to attend a two- or four-year college with a full course
of study. Applicants must also reside within the geographic
boundaries of a participating Chapter that offers scholarship
under the RMHC National Scholarship Program.
Application Requirements
Eligible students must submit a complete application
and attach all of the required documentation or their
application will not be reviewed. Applications
must be postmarked no later than February 15, 2007.
Scholarship Terms & Conditions
Most Local Chapters award a minimum of $1000, although
amounts may vary in some Program areas. Applicants should
contact their corresponding local Chapter for detailed

Scholarship recipients must enroll in and attend a
two- or four-year accredited institution in the academic
year following their selection. Verification of enrollment
is required. Scholarship funds will be paid via check
directly to the accredited institution in which the
student enrolls. Scholarship funds will not be paid
directly, nor reimbursed, to an award recipient. Scholarship
funds will be applied toward tuition, fees, and other
appropriate educational expenses, as determined by RMHC.
Supporting Documents - Transcript,
Personal Statement, Letter of Recommendation, Parent/Guardian's
IRS 1040 Form REQUIRED
A certified high school transcript that
contains the student's class rank and test scores must
be submitted with the application.
Applicants must submit a personal statement
that provides information about their community involvement,
career goals and desire to contribute to their community.
Information about unique personal or financial circumstances
may be added. Personal statements must not be more than
two pages in length.
A one-page letter of recommendation from
one of your teachers or school official is required.
The letter should detail the applicant's background,
achievements, leadership abilities and community involvement.
Letters should contain personalized information about
the applicant. Form letters will be given minimum consideration.
Applicants must submit a copy of their
parent's or guardian's IRS 1040 Form used to document
total income.
Certification & Release
All applicants and, where appropriate, their parent
or guardian, must sign the application, certifying that
all information provided is true and complete to the
best of their knowledge. Upon submission of the completed
application, applicants also grant RMHC and McDonald's
the right to use any information contained in the application
for the purpose of promoting and publicizing the Program,
or as legally required or permitted by law.
Submission of the Application
All complete applications must be postmarked by February
15, 2007. Incomplete application packets will not be
considered. Applications must be sent to the address
below. All documents must arrive in the same package
for the submission to be considered complete.
Program Administration
To assure complete impartiality in selection of winners
and to maintain a high level of professionalism, the
program is administered by Scholarship Program Administrators,
Inc., a firm that specializes in managing sponsored
scholarship programs.
